The failable questlines
These four threads have real failure states. Rule of thumb for all of them: check on the character after every major boss, and never leave an area “for now” with their thread mid-step.

Easily-missed one-off events
Not questlines — just moments that require being in the right place with the right state:
- Leaf’s Ring from Mary Ann (37.2%) — the game’s most important callback item, missed by nearly two-thirds of players. Talk to Mary Ann repeatedly as the story advances.
- Queen’s Tarts → Lorina (38.2%) — find the tarts, remember to deliver them.
- Humpty Dumpty’s Chick (34.8%) — requires deliberately feeding him souls; budget for it.
- The Duchess events — croquet hint (23%), the Walrus’ Dwelling encounter (26.4%).
- Hlanith Wine from Mabel in Winterbell’s wine cellar (18.1%).
- Florence + Blackwell’s Letter (16.7%) — the letter must be delivered after defeating her.
- Carnacki in Winterbell’s Throne Room (14.6%) — and choosing to clasp his shoulder.
- The snowman-maker’s identity (“Mysterious Sender”, 14.4%).
- Leiden at the Lion’s Fort (12%) — requires saving her from Mabel first.
The witnessing tier (under 12%)
The rarest achievements in BLACK SOULS II are not fights — they’re sights. Each requires specific, unprompted curiosity:
- The Two Birds (10.7%) — Jubjub meets the Chick at Carroll River; requires keeping the Chick’s storyline alive.
- Like a Fairy-Tale (9.6%) — talk to Kuti after she floods the Beach of Grief.
- Blank White Paper (9.4%) — the blank Fairy Tale.
- S T A R (9.2%) — the Dormouse’s prison event.
- “…The Lion beat the Unicorn all round the town.” (8.6%) — spare Unis after her betrayal. The single rarest tracked act in the game: almost everyone kills her.
How to actually not miss things
- Post-boss circuit. After every named kill, walk your list of live characters. Five minutes per circuit; it protects four questlines at once.
- Deliver items the day you get them. Tarts, letters, soup — courier quests in this game fail quiet.
- One cycle, two or three threads. Trying to babysit everything at once is how the Gerda/Kai thread dies while you’re watching Vernai.
